Where is your school, students, teachers and curriculum at now?
Outside of the triangle, write how the students are learning the curriculum between students and curriculum. Write how the teachers are teaching the curriculum and the role they are in now between teachers and curriculum. Write how the teachers reach the students and how students react to the teachers between teachers and students. Inside the triangle, give a picture of where your school is now. EX: partners, parent involvement, scheduling, etc. Problems may show up somewhere on this form also to be used later when developing your plan.
